I love receiving homemade gifts and cards from the kids on Mother’s Day. The only problem is, I’m the crafty one in our house. It’s not as much fun helping them make a gift for myself and it kind of takes the element of surprise (the kids’ favorite part) out of it.

I initially made this printable in 2016 but my boys thought it was too fancy and didn’t want to fill it out, so I made a less fancy form that hopefully everyone will love.
I always look forward to seeing what my kids write on projects like this. They say the funniest things and can be brutally honest. Our boys have done some similar projects at school. At Thanksgiving, they usually do a What are You Thankful For project. The oldest always put brother or grandparents. The middle child put mommy this year (yup I was on cloud nine). They have also done, What Do you Want to be When You Grow Up. I love reading their answers, seeing their handwriting, how they spell their words (at times trying to decode their words).
I think it would fun to make this All About My Mom survey as an annual Mother’s Day tradition and compare how much the kids have changed each year.
If your kids are not writing yet, they can dictate their answers to an adult or older sibling.
